The form is a PDF that can be completed on-line, printed and emailed to
CLA. Any individual travelling for CLA must complete and submit this
form to CLA.
CLA Expense Form
CLA's Expense Form is now available here in various formats.
The Microsoft
Excel version allow you to key in your expense information
without having to worry about the math. For those who wish, the form is
also available as an Adobe
Acrobat pdf file. Please note that since original receipts must
be submitted with your completed report, these cannot be faxed back to
the CLA office.
To access the Microsoft Excel, right click on the links above and
save the file on your hard drive. Start the appropriate software package
and open the saved file.
